At United Studios of Self Defense (USSD), we value your privacy and are committed to protecting your personal information. This policy applies specifically to residents of the State of Washington and those whose consumer health data is collected in Washington, in accordance with the Washington My Health My Data Act (MHMDA), and serves as an addendum to our regular Privacy Policy.

Categories of Consumer Health Data Collected

In order to provide safe and effective martial arts instruction, we may collect information that identifies your past, present, or future physical health status. This includes:

  • Health and Fitness Information: Physical fitness levels, weight, height, or goals.
  • Injury & Medical Conditions: Information regarding past injuries, surgeries, or chronic conditions (e.g., asthma, heart conditions) that may affect your ability to train.
  • Biometric Data: If used for facility access or progress tracking (e.g., fingerprints or body composition scans).
  • Inferred Data: Any health-related information we might infer based on your participation in specific specialized training programs.

Purpose of Collection and Use

We may use the information we collect for the following purposes:

  • To ensure your safety and provide appropriate instruction during physical training.
  • To provide the specific services you have requested (e.g., a personalized training plan).
  • To communicate with you regarding your health and safety while at our studio.

Sharing of Consumer Health Data

We do not sell your consumer health data. We only share this data with:

  • Service Processors: Trusted third parties who help us manage our studio operations (such as member management software), provided they adhere to our strict privacy standards.
  • Legal Requirements: When required by law or to protect the safety of our students and staff.
  • Consent: We will obtain your separate, affirmative consent before sharing your health data for any other purpose.

Your Rights Under MHMDA

You have specific rights regarding your health data, including:

  • Right to Access: You may request confirmation of whether we are collecting, sharing, or selling your health data.
  • Right to Withdraw Consent: You may withdraw your consent for our continued collection or sharing of your health data at any time.
  • Right to Deletion: You may request that we delete your consumer health data from our records, including backups and archives.
  • Right to Appeal: If we deny a request to exercise your rights, you have the right to appeal our decision.

How to Exercise Your MHMDA Rights

To exercise any of these rights, please submit a written request to us. We will respond to your request within 15 days. If your request is denied, you may appeal by contacting us at the same email; if your appeal is unsuccessful, you may contact the Washington State Attorney General.

We reserve the right to update this USSD MHMDA Policy from time to time. When we do, we will post the updated version on this page with a revised effective date. Please review this Policy periodically to stay informed about how we are protecting your information.
